Our Assist-Life Stage Transition program provides both short- and long-term assistance with living at home, community participation, and assisting you in coordinating your own support. This includes the following:
- Support connection and coordination
- Assistance with accommodation, tenancy obligations
- Mentoring, peer support
- Individual skill development
- Assistance with decision making and planning
